After reading the following, please contact our Billing Department at our toll free number, (877)347- 0992 with any questions you may have about billing and insurance.

If you have Health Insurance….

We are participating physicians with many health insurance companies in this area. We will be pleased to confirm your coverage and determine what deductible amounts or co-pays apply to the medical and/or surgical services we provide. Please understand that because of our contracts and state law, we must attempt to collect all appropriate funds for which you are responsible.

If we are not participating physicians with your health insurer, we will work with you to determine what your out-of-plan benefits are. In many cases, your insurer may pay more for services or a procedure performed by an out-of-plan provider than they pay for like services from a participating provider. We will, of course, look to you for payment first, then help you in any way we can with complete billing papers you can forward to your insurer for prompt reimbursement. Advance deposits may be required prior to the performance of endoscopic procedures.

If your health plan requires a Referral for Specialist Care, please bring the referral with you at the time of your office visit or procedure. If you fail to bring a required referral with you, you will be asked to sign a waiver form promising personal payment to us. We will discard this waiver once you present the proper referral form so we may successfully bill your insurer. If you do not present the required referral form and refuse to sign the waiver, we will regretfully have to reschedule your appointment to a later date.

Whether we are participating physicians with your insurer or not, we are most eager to be of service to you, and will do our best to help you through the health insurance maze. We will, no doubt, need your help with some of the process to be certain that you and our practice receive the reimbursement provided by your plan.

 
If you do not have Health Insurance….

Self-pay patients are welcome. As with insured patients, we will be pleased to work with you in making payment arrangements. Advance deposits will be required for endoscopic procedures.

Billing for Endoscopic Procedures….

If you have an endoscopic procedure, you will, of course, receive a bill from us for physician services. In addition, you will receive a bill from the facility (endo center or hospital), and, if tissue samples were obtained, another bill for the pathology services. These other bills are not processed through our practice, but our Billing Office will be happy to provide you with telephone numbers of the other billing offices.
